Medical Information
Medical Evaluation - DMV has a five-page medical form that is designed to be filled out by your doctor. Most of the requested information consists of short responses with a few places for more detailed comments. Of particular importance are the questions:
- Would the side effects from medication interfere with the safe operation of a motor vehicle?
- In your opinion, does your patient's medical condition affect safe driving?
- Have you advised against driving?
The medical evaluation is typically required for any medical condition, physical or mental. You may provide actual medical records or a letter from your doctor instead of the DMV form, but more is needed than simply a statement that “Patient is OK to drive.”
NOTE: Refusal to provide medical information may result in suspension (Vehicle Code §13801).
